Each household member enrolls at their own rate. Additional adult, teen, and child rates require a shared household address. The Student rate requires proof of enrollment each year. Membership is HSA-eligible. See the FAQ.

How Membership Works

Enrollment

Separate from your monthly rate, enrollment covers the time Dr. Stephens spends reviewing your records and getting ready to care for you.

Cancel Anytime

Membership is month-to-month, with no long-term contract. Cancel with 30 days’ written notice.

Need to Pause?

Pause once in any 12 months, for up to 30 days, with no dues. Your place is held.

Rejoining

After a 3-month wait, you can rejoin and pay the one-time enrollment again, if there is room.
